Instructions for filling out Housing Support Services Client Log and Invoice
Service Data:
1. Fill out the sheet that is named “Service Data” first and the information will populate
into the sheet that is named “Invoice.”
2 In the “Service Data” sheet fill out the columns:
* Service Date (exact date)
* Provider Code (we will give that you if you do not know it)
* Client Info: Name Last, Name First, Address
*Misc Items: Write in the item you purchased on behalf of a client for this contract.
(example: mattress cover). Write the purchase amount in the last column called
“purchases.”
